Baxter’s neuritis, also known as Baxter’s nerve entrapment or inferior calcaneal nerve entrapment, is a condition in which a small nerve in the foot—called the first branch of the lateral plantar nerve—becomes compressed or irritated. This nerve runs along the inner heel and supplies sensation to the heel area as well as motor control to some of the small muscles of the foot. When it becomes trapped between surrounding structures such as muscles, fascia, or bone, it can cause sharp, burning, or aching pain in the heel. Because its symptoms often resemble plantar fasciitis, Baxter’s neuritis is frequently misdiagnosed or overlooked.
The main causes of Baxter’s neuritis are related to mechanical compression and repetitive strain. Common factors include overpronation, flat feet, tight calf muscles, and running or standing for long periods. In athletes, especially runners, repetitive stress and poor footwear can lead to thickening of nearby tissues like the plantar fascia or abductor hallucis muscle, which can press against the nerve. In some cases, a bone spur on the heel (calcaneal spur) may also contribute to compression. These structural and functional issues combine to irritate the nerve, leading to chronic heel pain that worsens with activity and may persist even at rest.
The symptoms of Baxter’s neuritis typically include pain along the inner side of the heel that may radiate toward the arch or outer edge of the foot. Unlike plantar fasciitis, where pain is most severe with the first steps in the morning, Baxter’s neuritis pain often increases throughout the day or during prolonged standing. Some individuals describe tingling, numbness, or a burning sensation rather than a sharp stabbing pain. Diagnosis can be challenging and usually involves a combination of physical examination, nerve conduction studies, and imaging such as MRI or ultrasound to rule out other causes of heel pain.
Treatment for Baxter’s neuritis focuses on relieving nerve compression and addressing underlying biomechanical factors. Conservative management includes rest, stretching exercises for the calf and plantar fascia, custom orthotics to improve foot alignment, and anti-inflammatory medications. Physical therapy can help reduce muscle tightness and improve foot mechanics. In persistent cases, corticosteroid injections may be used to decrease inflammation around the nerve. If conservative care fails, surgical decompression may be necessary to release the entrapped nerve. Early recognition is crucial, as timely treatment can relieve pain and prevent long-term nerve damage, allowing patients to return to normal activity comfortably..
