Heel pain is a common complaint and can arise from a variety of causes, often depending on activity level, age, and biomechanics. One of the most prevalent causes is plantar fasciitis, which results from inflammation of the plantar fascia—a thick band of connective tissue that runs from the heel bone (calcaneus) to the toes. This condition typically causes a sharp, stabbing pain that is most noticeable with the first steps in the morning or after prolonged periods of rest. It’s commonly associated with overuse, flat feet or high arches, and poor footwear.
Another significant cause is Achilles tendinitis, an overuse injury of the Achilles tendon, which connects the calf muscles to the heel. This condition typically develops gradually, with dull pain or stiffness along the back of the heel that worsens with activity. It’s particularly common in runners or people who suddenly increase their activity level. Contributing factors include tight calf muscles, improper training techniques, and lack of adequate warm-up or stretching.
Heel spurs are also a well-known source of heel pain, although they’re often misunderstood. A heel spur is a bony outgrowth on the underside of the heel bone, frequently found in conjunction with plantar fasciitis. While not always painful, when pain does occur, it is usually due to associated soft tissue inflammation rather than the spur itself. Heel spurs are often the result of repetitive stress, poor walking mechanics, or excessive weight bearing.
Bursitis and nerve entrapments can also lead to heel discomfort. Retrocalcaneal bursitis occurs when the bursa (a fluid-filled sac) located between the Achilles tendon and the heel bone becomes inflamed, often from repetitive motion or pressure. Meanwhile, conditions like tarsal tunnel syndrome—where the posterior tibial nerve is compressed—can cause heel pain accompanied by tingling, numbness, or burning sensations. These nerve-related issues often stem from structural problems or injuries and may require more specialized evaluation for proper management.
