Forefoot Valgus

Forefoot valgus is a structural foot deformity characterized by the lateral (outward) deviation of the forefoot relative to the rearfoot when the subtalar joint is in a neutral position. In this condition, the lateral side of the forefoot is lower than the medial side, making the first and second metatarsals sit higher off the ground than the fourth and fifth when the heel is in neutral alignment. It’s important to differentiate forefoot valgus from forefoot varus, as the treatment approach can be quite different. The condition can be either rigid (fixed bony deformity) or flexible (soft tissue adaptation), with the rigid type generally being more symptomatic and requiring more aggressive intervention.

From a biomechanical standpoint, forefoot valgus can have significant consequences. When walking, the body compensates for the tilted forefoot by causing the rearfoot to invert during midstance and propulsion to allow full forefoot contact with the ground. Over time, this compensation can lead to various issues like ankle sprains, peroneal tendonitis, or lateral knee pain, especially in athletic or high-demand populations. It also contributes to abnormal pronation-supination patterns, increasing the risk for overuse injuries. In some cases, it may even lead to a functional pes cavus (high arch), as the foot attempts to maintain balance and propulsion through altered mechanics.

Treatment of forefoot valgus depends heavily on symptom severity, functional impairment, and rigidity of the deformity. Custom orthotics are a common non-surgical intervention, specifically designed with a lateral forefoot post to bring the ground up to the forefoot and avoid rearfoot compensation. For rigid deformities, especially those that cause persistent symptoms or secondary issues like lateral foot pain or instability, surgical correction (e.g., osteotomies or joint fusions) might be considered. However, conservative management is usually effective if caught early, making proper biomechanical assessment and early diagnosis key to preventing complications..
